Job summary

MLK Community Healthcare is seeking a Care Coordinator for Enhanced Care Management (ECM) to act as a liaison among patients, providers, and the healthcare system in Los Angeles, CA. The role involves discharge planning, care coordination, and ensuring adherence to care management criteria across services.

Responsibilities include home visits, patient advocacy, and facilitating access to community-based services.

Qualifications

• One (1) year of customer service experience in a healthcare setting or similar preferred.
• Valid unrestricted CA Driver’s License with proof of vehicle insurance; must present within 30 days of hire/transfer.
• A team player that can follow a system and protocol to achieve a common goal.
• Must complete annual Workplace Violence Prevention Program/Certificate within 30 days of hire/transfer.
• Highly organized with strong oral and written communication skills.
• Confidence to communicate with community health care organizations and personnel.
• Demonstrates sound judgment, decision making and problem solving skills.
• Prior Cerner EMR experience preferred.
• Prior healthcare facility or Street Medicine experience preferred.

Responsibilities

• Participates in implementing the ECM program to ensure eligible Medi-Cal beneficiaries receive enhanced care management and coordination.
• Supports patient self-management and promotes behavioral modifications.
• Conducts home visits and street visits for enrollment and ongoing services.
• Assists patients as a health systems navigator and advocate.
• Helps patients access community-based services and supports.
• Coordinates continuity of patient care with external healthcare organizations and facilities.
• Coordinates care following hospital admission, discharge, and ED visits.
• Reports care barriers to physicians or designee.
• Performs preventive screenings and supports daily patient interactions as needed.
• Facilitates communication among interdisciplinary care team members regarding patient care plans.
• Manages patient medication coordination based on standing orders.
• Supports data collection, health outcomes reporting, and clinical audits.
• Assists in developing new clinical tools, forms, and procedures.
• Works with leadership on project plans, timelines, and risk mitigation.
• Provides direction to project team and identifies risks with solutions.
• Projects defined: Metrics, Physician Dashboards, Quality Metrics.
• Assists in creation/submittal of Medical Group invoices.
• Travels to MLKCH ambulatory, hospital, and admin sites as needed.
