Venice Family Clinic is seeking a Lead Care Manager for its Enhanced Care Management (ECM) program on the Westside of Los Angeles. The role provides high-quality case management to medically and socially complex patients with guidance from the Care Management Director.

Qualifications

• Bachelor’s degree in social work and 1 year of relevant experience or equivalent.
• Knowledge of community resources, health education, advocacy, harm reduction, and general clinical knowledge.
• Ability to travel within Los Angeles County for outreach.
• Ability to complete required program documentation.
• Ability to set boundaries with clients and discuss sensitive topics non-judgmentally.
• Strong organizational skills to manage deadlines and multiple tasks.
• Ability to work independently and with diverse populations.
• Bilingual English and Spanish skills preferred.

Responsibilities

• Provide case management to medically and socially complex high-risk individuals.
• Perform outreach and enroll clients in ECM.
• Conduct comprehensive assessments and develop individualized care plans.
• Provide interventions and accompany clients to scheduled medical appointments.
• Maintain accurate charts and documentation.
• Advocate with primary care providers to ensure access to quality care.
• Collaborate with Medical Care Coordination and participate in multidisciplinary meetings.
• Attend clinical conferences and assist with reports.
