Job summary

South-Central Family Health Center seeks a Lead Care Manager for the Enhanced Care Management program in Los Angeles. The role coordinates ECM services, develops personalized care plans, and collaborates with medical, dental, behavioral health, and vision teams to improve patient outcomes.

The ideal candidate will have 2+ years of case management experience in a community clinic, bilingual English/Spanish, and strong communication skills. Full-time exempt position with competitive benefits.

Qualifications

• High School Diploma or equivalent required.
• Bachelor’s Degree preferred but not required.
• Medical Assistant or LVN certification preferred.
• Bilingual English/Spanish preferred.
• Minimum 2 years’ experience in case management in a community clinic or FQHC.

Responsibilities

• Oversees and implements ECM services and care plan goals.
• Develops personalized care plans with member input.
• Coordinates with ECM partners and resources.
• Provides translation for staff when necessary.
• Maintains HIPAA and confidentiality standards.
