Job summary

Los Angeles Christian Health Centers is seeking an Outreach Care Coordinator to lead care management for Medi-Cal ECM participants, coordinating biopsychosocial services and promoting community engagement. The role involves outreach, enrollment, and collaboration with partners to improve patient access and outcomes.

The OCC will manage ECM caseloads, apply evidence-based interventions, and support self-management for patients.

Qualifications

• CPR Certification required
• Bi-lingual Spanish preferred
• Two years of case management experience is required
• Bachelor's degree in social work or related field preferred
• Experience with homeless populations and complex social needs is beneficial
• Proficiency with common clinic software and databases is preferred

Responsibilities

• Build and maintain partnerships to enhance services for patients
• Represent LACHC at community and faith-based events
• Provide ECM care coordination and lead in ECM activities
• Coordinate referrals, follow-ups, and care transitions
• Assist patients with enrollment and access to resources
• Utilize assessments to inform care plans and track progress
• Support outreach and enrollment of new patients
