Job summary

MLK Community Healthcare seeks a Care Coordinator for Enhanced Care Management (ECM) to serve as a liaison among patients, providers, and the healthcare system in Los Angeles, CA. You will implement ECM, promote self-management, and ensure seamless discharge planning and medication coordination.

The role emphasizes patient advocacy, navigation, and collaboration with interdisciplinary teams to improve health outcomes. Travel to ambulatory, hospital, and administrative sites is required as needed.

Qualifications

• One year of customer service experience in a healthcare setting.
• Must have valid CA driver's license with proof of vehicle insurance.
• Must complete Workplace Violence Prevention Program within 30 days of hire.
• Excellent communication, organization and problem-solving skills.
• Experience with healthcare systems and care coordination is preferred.

Responsibilities

• Participates in implementing the Enhanced Care Management (ECM) program to ensure eligible Medi-Cal beneficiaries receive ECM.
• Supports patient self-management and behavioral modification interventions.
• Conducts home visits and street visits for enrollment and ongoing services.
• Acts as patient advocate and health system navigator for patients.
• Assists patients in navigating community-based services to support healthy living.
• Coordinates continuity of care with external healthcare organizations and facilities.
• Gathers data for health outcomes reporting and clinical audits.
• Contributes to project planning, timelines, and risk resolution.
• Travel to MLKCH sites as needed to aid patient care.
