Job summary

BLEHEALTH, LLC is seeking a Lead Care Manager to coordinate Enhanced Care Management (ECM) for chronically ill and high‑risk members in the South Los Angeles area. You will partner with members, families, providers, hospitals, and community resources to deliver person‑centered care.

The role requires strong care coordination, scheduling, and documentation skills, with responsibility for engaging members in their local communities and supporting access to medical and behavioral health services.

Qualifications

• Valid California driver’s license and clean driving record
• Reliable personal vehicle and ability to drive within a 20‑mile radius
• Live Scan fingerprint/background check required
• Negative TB test and current CPR certification
• Ability to meet daily productivity expectations
• Experience with ECM or Medi-Cal programs is a plus

Responsibilities

• Coordinate care across clinics, hospitals, and community agencies to ensure seamless service delivery.
• Oversee ECM services and implement individualized care plans.
• Provide services where the member lives or seeks care.
• Assess medical, behavioral, and social needs and develop comprehensive care plans.
• Assist with transportation and appointment scheduling.
• accompany members to office visits when appropriate.
• Monitor treatment adherence and medication compliance.
• Deliver health education using motivational interviewing and trauma-informed care.
