Job summary

Horizon Recuperative Care is seeking an Enhanced Care Management (ECM) specialist to engage and support members in the ECM program at our Los Angeles facility. You will monitor adherence to care plans, provide health promotion, and coordinate with discharge planning and community providers.

Required CalAIM experience, a related field certificate, at least one year in community living, and strong English communication. HIPAA compliance and independent work are essential.

Qualifications

• CalAIM initiative experience.
• Certificate in a related human/social service field.
• At least 1 year of recent community living experience.
• Strong written and interpersonal English communication.

Responsibilities

• Engage and support ECM program members.
• Monitor treatment adherence via care plans aligned with provider recommendations.
• Conduct health promotion and self-management training.
• Maintain regular communication with members, including home visits if necessary.
• Build and nurture relationships with internal and external stakeholders.
• Assist with discharge planning and transition of care.
• Coordinate care with case management staff and community providers during emergencies.
• Attend medical appointments to ensure continuity of care.
• Document interactions and maintain records of referrals to various support services.
• Conduct psychosocial assessments and develop treatment plans collaboratively.
• Perform other ECM-related duties as assigned.
• Ensure HIPAA compliance and treat member information confidentially.
• Adhere to local, state, and federal regulations.
• Contribute to quality assurance/improvement activities.
• Utilize Electronic Health Record systems efficiently.
