Job summary

Horizon Recuperative Care in Los Angeles, CA seeks an ECM to engage and support members, monitor treatment plans, and coordinate care with clinicians and community providers. This role emphasizes patient-centered care in a restorative setting.

Qualifications

• CalAIM experience in Medi-Cal initiatives is required.
• One year of related experience in community living.
• Strong written, oral, and interpersonal English communication.
• Knowledge of behaviour management and positive role modelling.

Responsibilities

• Engage and support ECM program members.
• Monitor adherence to care plans and coordinate with providers.
• Conduct health promotion and self-management training.
• Communicate regularly with members; perform home visits if needed.
• Coordinate discharge planning and transitions of care with staff.
• Document interactions and referrals in records system.
• Ensure HIPAA compliance and follow agency policies.
