Horizon Recuperative Care in Los Angeles, CA is seeking an Enhanced Care Management (ECM) professional to support recovering individuals through coordinated care plans and community resources. In this role you will engage members, monitor adherence, provide health promotion and self-management training, and work with discharge planning, while maintaining HIPAA and collaborating with providers.

Qualifications

• CalAIM initiative experience in California.
• Certificate or equivalent in a related human/social service field.
• One year of recent related experience in community living.
• Demonstrated written, oral and interpersonal communication skills in English.
• Knowledge of and experience in Behaviour Management.

Responsabilités

• Engage and support members in the ECM Program.
• Monitor treatment adherence through care plans.
• Conduct health promotion and self-management training.
• Maintain regular communication with members, including home visits if necessary.
• Coordinate care with case management staff and community providers during emergencies.
• Document interactions and maintain records of referrals.
• Ensure HIPAA compliance and confidentiality.
• Adhere to local, state, and federal regulations.
