Job summary

L.A. Care Health Plan in Los Angeles, CA is hiring a Manager for Enhanced Care Management (RN or LCSW). The role oversees ECM staff, ensures regulatory compliance, and collaborates with hospitals, providers, and community-based organizations to deliver high-quality care.

Requires leadership experience and relevant licensure. Full-time position located in Los Angeles with a salary range reflecting elasticity across California's public health programs.

Qualifications

• Requires Masters in Social Work or an Associate's or Bachelor's degree in nursing and related licensure.
• Minimum 6 years of care management experience in managed care or equivalent settings.
• At least 4 years leading a program, staff, or supervisory responsibilities.
• Knowledge of CalAIM ECM principles, policies, and processes.

Responsibilities

• Oversee ECM Provider activities and ensure compliant performance of contracted activities.
• Coordinate training topics and Joint Operating Meetings with staff.
• Ensure high-quality ECM services and establish QA mechanisms and timely intakes.
• Lead program operations enhancements and ensure state/federal compliance reporting.
• Engage with community agencies and partners to improve ECM services and manage staff performance.
• Maintain HIPAA confidentiality and uphold reliability and attendance standards.
• Perform other duties as assigned.
