Qualifications

• 3+ years of ECM, case management, social services, or healthcare coordination.
• Strong mentoring, training, or staff support experience preferred.
• Excellent communication, leadership, organizational, and problem-solving skills.

Responsibilities

• Provide day-to-day support and guidance to Lead Care Managers (LCMs).
• Monitor productivity, documentation completion, and operational consistency.
• Assist with onboarding and staff development.
• Coordinate care across medical, behavioral health, and community services.
• Ensure documentation quality and regulatory compliance.
