Responsibilities

Assist individuals with personal care and daily routines per their support plan.
Transpose to community and activities while promoting self-direction.
Provide safe transportation using agency or personal vehicles as needed.
Manage resources and ensure appropriate use of funds, vehicles, and equipment.
Document shift notes and maintain electronic records per funding requirements.
Support the person with meals, daily living activities, and home maintenance tasks.
Prepare meals according to scheduled menus and encourage independence.
