Support Solutions Bolivar, TN seeks compassionate Direct Support Professionals to assist individuals with disabilities in daily living and personal care within home and community settings. You will document progress and follow ISP goals while helping clients develop essential skills and independence.

In this full-time role, you will transport when needed, uphold safety standards, and collaborate with teams.

Qualifications

• Must be 18 years of age and able to read and write effectively.
• ECF staff must have high school diploma or GED.
• Must have a valid drivers license and maintain a valid drivers license and acceptable driving record throughout employment with SS.
• Must meet state and local requirements regarding criminal background check, sexual offender check, abuse registry, drug and felony offender listings.
• Must meet all training requirements and keep training current.

Responsabilités

• Supports the person supported with personal care and daily routines, and in achieving outcomes from the ISP.
• Understands and practices Support Solutions philosophy and values in interactions with the person supported and community members while ensuring the person is empowered to make decisions.
• Helps the person supported become integrated with the community by providing person centered supports.
• Assures safe transport and accompanies the person supported as needed to events, job sites, and medical appointments using various transport means.
• Ensures proper use of agency resources including personal funds, vehicles, buildings, utilities, etc.
• Attends all training meetings assigned by Support Solutions.
• Cooks and provides meals based on scheduled menus.
• Performs daily activities or teaches the person supported to do so to foster independence.
• Consistently works and documents progress toward meeting outcomes.
• Utilizes electronic records system and completes daily shift notes as required.
• Ensures customer satisfaction by following Customer Service Standards.
• Oversees storage and administration of medications.
• Provides assistance with personal grooming and keeping house clean.
