Support Solutions in Bolivar, TN is seeking a dedicated Direct Support Professional to assist individuals with disabilities in daily living and community activities. Qualifications

• Must be 18+ years old and able to read and write effectively.
• ECF staff must have high school diploma or GED.
• Must have a valid driver's license and maintain driving record.
• Must meet state and local requirements for background checks and clearances.
• Must meet all training requirements and keep training current.

Responsibilities

• Supports the person supported with personal care and daily routines per the ISP.
• Respectfully interacts with the person supported and community members to uphold dignity.
• Helps the person supported integrate with the community through person-centered supports.
• Ensures safe transport using company or personal vehicles and alternative arrangements when needed.
• Documents daily shift notes and uses the records system to track progress.
