Qualifications

• Must be 18 years of age and able to read and write effectively.
• High school diploma or GED required.
• Valid driver license and clean driving record throughout employment.
• Meet state/local background checks and training requirements.
• Must keep training current and pass required certifications.

Responsibilities

• Supports the person with personal care and daily routines as per ISP.
• Helps the person become integrated with the community through centered supports.
• Provides safe transport to events, job sites, and medical appointments.
• Manages agency resources and documents care in electronic records.
• Attends trainings; follows customer service standards; supports independence.
• Assists with meals, housekeeping, and personal grooming as requested.
