Job summary

Support Solutions Smyrna Area in Smyrna, TN is seeking a part-time Direct Support Professional to empower individuals with disabilities and help them lead self-directed lives in a safe, supportive environment. The role involves assisting with personal care, daily living activities, transportation, documentation, and community integration.

A valid driver’s license, background checks, and training compliance are required.

Qualifications

• Must be 21 years of age and able to read and write effectively.
• ECF staff must have high school diploma or GED.
• Must have a valid driver’s license and maintain a valid driving record.
• Must meet state and local criminal background checks and training requirements.

Responsibilities

• Supports the person in daily routines and ISP-goal attainment.
• Provides safe transport to events, jobsites, and medical appointments.
• Maintains confidentiality of clients and uses electronic records as required.
• Cooks and provides meals based on scheduled menus.
• Documents daily shift notes and outcomes to funding entities.
