Qualifikationen

• Associate/Bachelor’s degree preferred.
• 3+ years of experience in case management or housing services.
• Fluency in Spanish is highly desirable.

Aufgaben

• Engage with tenants within 15 days of move-in.
• Complete needs assessments and develop support plans within 30 days and quarterly thereafter.
• Meet with tenants monthly with at least one home visit every three months.
• Assist tenants in accessing benefits, healthcare, mental health, employment resources, and other supports.
• Provide crisis intervention, conflict resolution, and mediation between tenants and staff.
• Build trusting relationships to foster independence and community engagement.
• Develop partnerships with local service providers and community programs.
• Connect tenants to educational workshops, support groups, and wellness activities.
• Collaborate with property management on unit conditions, safety, and lease compliance.
• Advocate for tenants with external agencies for necessary support.
• Maintain accurate case notes, service plans, and assessments.
