The role emphasizes delivering appropriate information and referrals to obtain housing and documenting activities per guidelines. The position requires a Bachelor's degree in Social Work or a related field, at least 1 year of case management experience, and strong organizational skills.

Qualifications

• Bachelor's Degree in Social Work or Human Services Field required.
• Proof of Education is required.
• At least 1 year of case management experience with homeless, mentally ill, substance abuse, and/or low-income populations.
• Bilingual a plus.
• Strong relationship-building skills required.
• Ability to work independently but know when to raise issues to the supervisor.
• Excellent organizational and time management skills are necessary.
• Ability to multitask and follow up in a timely manner.
• Strong PC skills, including Word and Excel. CARES experience preferred.

Responsibilities

• Conduct intake and assessment to orient clients to the program and develop an individual service plan.
• Meet weekly with clients to assist with the achievement of service plan goals.
• Coordinate services with Housing Specialist, other staff, and external partners.
• Provide advocacy for clients around entitlements and services.
• Provide crisis intervention and supportive counseling.
• Document case management activities per guidelines.
• Complete Housing applications for clients based on needs.
• Aim to achieve monthly placement quota.
• Facilitate workshops and escort clients as applicable.
• Attend staff meetings and trainings to enhance job skills.
