Job summary

The role includes weekly client meetings, coordination with Housing Specialist and partners, advocacy, and thorough documentation of activities while maintaining a 35-hour work week in Manhattan. A Bachelor’s degree in social work or related field is preferred with at least 1 year of case management experience; bilingual

Qualifications

• Bachelor's degree in social work or related field preferred.
• At least 1 year of case management experience with homeless, mentally ill, substance abuse, or low-income populations.
• Bilingual a plus; strong relationship-building and organizational skills; ability to work independently.

Responsibilities

• Conduct intake and assessment to orient client and develop an individual service plan.
• Meet weekly with client to help achieve service plan goals.
• Coordinate services with Housing Specialist and other staff/partners.
• Provide advocacy for entitlements and services.
• Provide crisis intervention and counseling to support decision making.
• Document case management activities per agency/funding guidelines.
• Complete housing applications for clients based on needs.
• Achieve monthly placement quota.
• Facilitate workshops/groups and escort clients as needed.
• Attend staff meetings and trainings; maintain flexibility for evenings/weekends.
