Job summary

Westhab Inc. is seeking a Housing Case Manager in White Plains, NY to provide housing-focused case management for homeless families in a Tier II emergency shelter.

The role involves coordinating with WCDSS, formulating housing plans, and guiding residents through leases, subsidies, and moving costs while maintaining HUD-compliant records. The position requires a bachelor’s degree in social work or related field, strong communication skills, and experience in case management or homelessness;

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Strong computer, presentation, written and verbal communication skills required.
• Must have experience in case management, home visiting, homelessness, or child welfare.
• Fluency in Spanish is highly desirable.

Responsibilities

• Works with Westchester County Department of Social Services (WCDSS) and other providers to help families achieve housing goals.
• Formulates a housing plan with residents to transition to permanent housing and manage subsidies.
• Provides tenant education on leases, rights and responsibilities, financial literacy and daily living skills.
• References residents to specific housing opportunities and assists with housing and rental subsidy applications.
• Maintains accurate documentation and records per HUD and WCDSS requirements.
