Job summary

Westhab Inc. is seeking a Case Manager in the Bronx to assess clients, plan services, and connect residents with housing resources.

The role involves coordinating with Housing Specialists and community partners to ensure timely referrals and progress toward individual service goals. The position requires a Bachelor’s degree in Social Work or related field, at least one year of case management experience with vulnerable populations, and strong organizational skills.

Qualifications

• Bachelor's Degree in Social Work or Human Services; Proof of Education required.
• At least 1 year of case management experience with homeless, mentally ill, substance use, or low-income populations.
• Bilingual a plus; strong relationship-building and independent work abilities.
• Excellent organizational and time management skills; able to multitask and follow up timely; strong PC skills (Word, Excel). CARES experience preferred.

Responsibilities

• Conduct intake and assessment to orient clients and develop an individual service plan.
• Meet weekly with clients to help achieve service plan goals.
• Coordinate services with Housing Specialist and external partners; provide advocacy on entitlements.
• Provide crisis intervention and supportive counseling; document activities per guidelines; assist with housing applications.
• Attend staff meetings and trainings to enhance job skills; accommodate flexible schedules including evenings/weekends.
