Job summary

Elite Home Health, a part of LHC Group, a leading post-acute care partner for hospitals, physicians and families nationwide, is seeking a Physical Therapist Assistant. You will perform treatment and provide exercise instruction and patient education to improve well-being according to the plan of care.

As part of UnitedHealth Group's mission to help people live healthier lives, you will collaborate with supervising PTs, nurses, and other team members to deliver high‑quality home health services

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance or access to dependable transportation.
• Ability to function in any home situation regardless of client demographics.
• Available to work a weekend rotation.

Responsibilities

• Follow state laws governing physical therapy in home care and supervise plan of care defined by supervising PT.
• Instruct and aid patients in active and passive exercise, gait, and ADL training.
• Observe, record, and report patient responses to treatment to supervising PT and other team members.
• Instruct patient, family, and other care team members in therapy within the scope of PT assistant.
