Job summary

Elite Home Health, part of LHC Group and Optum family, is seeking a Physical Therapist Assistant to carry out treatment and educate patients under the established plan of care. You will guide exercise, gait, and functional activities while coordinating with the supervising Physical Therapist and nursing staff.

Responsibilities include documenting patient response, collaborating with the healthcare team, and educating families and caregivers.

Qualifications

• Active CPR certification in good standing.
• Current license or certification as a Physical Therapist Assistant.
• Access to a dependable vehicle or reliable transportation.

Responsibilities

• Follow state laws and stay within plan of care under supervising PT.
• Instruct patients in active and passive exercise and mobility tasks.
• Monitor patient progress and communicate changes to the PT and care team.
• Educate patients, families, and caregivers on therapy techniques.
