Job summary

Elite Home Health, part of LHC Group and a member of the Optum family, seeks a Physical Therapist Assistant to deliver therapy services in home care following a supervised plan of care and promoting patient well‑being.

Responsibilities include following state laws, guiding exercises, observing patient progress, and educating patients and families. Weekend rotation is required. Current PTA licensure and CPR are required; pay range is listed below.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current CPR certification
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
• Available to work a weekend rotation

Responsibilities

• Follow state laws governing provision of physical therapy in home care and implement plan of care defined by supervising PT.
• Instructs and aids patients in active and passive exercise, gait, functional activities, ADL, transfers, safety, and prosthetic training
• Observes, records, and reports patient responses; coordinates care with the healthcare team
• Instructs patient, family, caregiver in therapy within scope of PTA
