Job summary

Qualifications

• ACOTE-accredited OT program required.
• Active state OT license required.
• NBCOT certification eligibility or certification completed.
• Current CPR certification required.
• 1+ year OT experience required; mentoring/residency if less than 1 year may be required.

Responsibilities

• Deliver OT services per physician-approved plan of care.
• Ensure plan of care addresses identified problems and document rationale.
• Ensure safety and effectiveness of OT service delivery.
• Complete documentation timely and accurately.
• Supervise home health aide when RN is not active in the case.
• Perform other duties as assigned.
