Job summary

Hope-Haven-Inc. in Iowa is seeking dedicated caregivers to support individuals with disabilities per individualized service plans. Responsibilities include assisting with daily living activities, hygiene, and mobility support in a respectful, person-centered environment.

Position requires strong communication, reliability, and the ability to coordinate transportation and appointments, with emphasis on safety and teamwork within the care team.

Qualifications

• Good written and verbal communication skills.
• Ability to exercise sound judgment in decisions.
• Reliable transportation.

Responsibilities

• Support individuals as identified in the service plan, assisting with hygiene, bathing, toileting, escort, transition time, and mealtime assistance.
• Provide training and supervision per service plan to ensure physical and mental needs are met.
• Teach self-control and self-management per departmental training.
• Complete client logs and charts within 24 hours with supervisor approval.
• Assist individuals in developing daily activities tailored to their needs.
• Arrange appointments and transportation; ensure documentation is complete.
• Promote safety and follow all procedures.
• Be a team player, communicating effectively with all staff and the community.
