Qualifications

• High School Diploma required.
• Good communication (verbal & written).
• Driver's license and background checks required.
• Basic computer skills; willingness to learn Excel.

Responsibilities

• Support individuals in community settings and daily living.
• Provide transportation to activities and appointments.
• Assist with personal care as needed per plan.
• Document client logs and charts promptly.
• Promote safety and teamwork among staff and clients.
• Monitor alerts and provide virtual support as scheduled.
• Help develop and carry out daily activity schedules.
• Coordinate with team for emergencies and transportation.
• Encourage community integration and participation.
• Complete trainings within established timelines.
