You will provide care and training per each client’s plan, help with activities, personal care, transportation, and ensure accurate shift documentation while prioritizing safety and dignity. Strong communication, reliability, teamwork, and a compassionate approach are essential to

Qualifications

• High school diploma or GED required.
• Good written and verbal communication; basic computer skills.
• Ability to exercise sound judgment.
• Must complete medication administration course.
• Valid Class C driver’s license.
• Background checks clear per service standards.
• Reliable transportation and proof of state minimum liability insurance.

Responsibilities

• Support individuals according to service plans and meet physical, mental, and social needs.
• Provide care and training per service plan, including hygiene, bathing, toileting, escort, transition time, and mealtime assistance.
• Teach self-control and self-management skills per departmental training.
• Complete client logs and charts within 24 hours with supervisor approval.
• Assist individuals in developing and carrying out a daily activities schedule tailored to their needs.
• Arrange necessary appointments and transportation and ensure proper documentation.
• Promote personal safety and follow all safety procedures.
