Qualifications

• High school diploma or GED/HSED required.
• Good written and verbal communication and basic computer skills.
• Ability to exercise sound judgment.
• Valid Class C driver's license; background checks clearance.

Responsibilities

• Support clients based on individualized goals to meet physical, mental, emotional, and social needs.
• Assist with hygiene, bathing, toileting, mealtime, and daily activities per service plan.
• Teach self-control and self-management skills per training.
• Complete client logs and charts within 24 hours with supervisor approval.
• Help clients arrange appointments and transportation with proper documentation.
• Promote safety and follow all safety procedures.
