Job summary

Woodward Skilled Nursing & Therapy is seeking a detail-oriented PPS Skilled Case Manager / MDS Coordinator (RN or LPN) to oversee the MDS process in a long-term care setting in Woodward, Oklahoma. This role ensures accurate assessments, care planning and regulatory compliance.

The position emphasizes interdisciplinary collaboration, timely documentation and payer coordination, with a focus on delivering resident-centered care within a non-exempt, FLSA status environment.

Qualifications

• Current Oklahoma RN or LPN license.
• 2+ years MDS/long-term care or case management experience.
• Knowledge of MDS/RAI process and care planning.
• Ability to develop comprehensive, individualized plans of care based on residents’ conditions.

Responsibilities

• Coordinate the complete MDS process, including scheduling, data collection, completion and electronic submission of assessments.
• Complete and coordinate required MDS assessments, including quarterly ones, within required timeframes.
• Ensure MDS assessments reflect residents’ medical condition, functional status, and care needs.
• Maintain MDS and Care Plan tracking for timely completion and compliance.
• Coordinate development of individualized, comprehensive resident care plans.
• Gather clinical information for skilled assessments, care planning, and payer requirements.
• Coordinate payer requirements with resident needs, treatment plans, and outcomes.
• Monitor residents and documentation to reflect services in the medical record.
• Communicate with nursing, therapy, providers, and interdisciplinary team.
• Participate in care plan decisions and ensure required documentation is completed.
• Record and transmit MDS data to state and federal databases.
• Work with the team to promote quality outcomes and resident-centered care.
