Job summary

Kingwood Skilled Nursing & Therapy in Oklahoma City is seeking a licensed nurse (RN or LPN) to join as an MDS Coordinator. The role orchestrates MDS, RAPs and care plans for residents, with emphasis on timely data collection and accurate documentation.

Current Oklahoma RN or LPN license is required; CPR certification and MS Office skills are expected. We offer comprehensive benefits and support professional growth for full-time staff.

Qualifications

• Current Oklahoma RN or LPN license and in good standing.
• Knowledge of MDS and RAPs; able to craft individualized care plans.
• Up-to-date CPR certification; strong data accuracy and documentation practices.

Responsibilities

• Collect data for MDS including quarterly MDS in a timely manner as required by the RAI manual and HCFA guidelines.
• Maintain accurate MDS/Care Plan tracking to ensure timely completion.
• Ensure each MDS is completed accurately reflecting resident medical status with proper documentation.
• Coordinate decisions about proceeding to care plan and document on MDS per state/federal regulations.
• Electronically record and transmit MDS data according to state database requirements.
