Job summary

Woodward Skilled Nursing & Therapy is seeking a detail-oriented PPS Skilled Case Manager / MDS Coordinator (RN or LPN) to oversee the MDS process and care planning for residents in our nursing facility in Woodward, Oklahoma.

You will coordinate assessments, ensure regulatory accuracy, and collaborate with an interdisciplinary team to deliver compliant, resident-centered care. Competitive compensation and benefits support ongoing professional growth.

Qualifications

• Oklahoma RN or LPN license in good standing.
• 2+ years experience with MDS, long-term care, or skilled nursing.
• Knowledge of MDS, RAI, care planning and LTC requirements.
• Ability to develop comprehensive, individualized care plans.

Responsibilities

• Coordinate and oversee the complete MDS process, including scheduling, data collection, completion, review, and submission.
• Complete and coordinate required MDS assessments, timely and compliant.
• Ensure MDS reflects residents' current medical condition and care needs.
• Maintain MDS and Care Plan tracking for timely completion and regulatory compliance.
• Coordinate development and completion of individualized, comprehensive resident care plans.
• Gather clinical information for skilled assessments, care planning, and payer needs.
• Coordinate payer requirements with resident needs, treatment plans, and outcomes.
• Monitor residents and supporting documentation to reflect services in the medical record.
• Communicate with the interdisciplinary team regarding resident needs and care planning.
• Record and transmit MDS data to state and federal databases as required.
• Collaborate with the interdisciplinary team to promote quality outcomes and resident-centered care.
