Job summary

Arbor Village Skilled Nursing & Rehabilitation in Sapulpa, OK is seeking a seasoned MDS Coordinator (RN or LPN) to join our nursing leadership team. You will oversee accurate CMS-compliant MDS assessments and coordinate OBRA and Medicare reviews to support resident care plans.

Qualifications

• Current RN or LPN license in good standing.
• MDS experience in a skilled nursing or long-term care setting.
• Strong knowledge of MDS 3.0, PDPM, CMS regulations, and the RAI process.
• Experience with PointClickCare (PCC) preferred.
• Strong organizational, communication, and leadership skills.
• Ability to manage deadlines, work independently, and collaborate with the interdisciplinary team.

Responsibilities

• Complete and coordinate MDS assessments accurately and within CMS-required timeframes.
• Ensure compliance with federal and state regulations, RAI guidelines, and facility policies.
• Manage the MDS assessment schedule, including OBRA and Medicare assessments.
• Collaborate with nursing, therapy, social services, dietary, and other departments to ensure accurate assessments and individualized care plans.
• Review clinical documentation to support accurate PDPM classification and appropriate reimbursement.
• MonitorQuality Measures (QMs), identify opportunities for improvement, and participate in QAPI initiatives.
• Assist with care plan development, updates, and interdisciplinary care conferences.
• Participate in Medicare triple-check processes and support billing accuracy.
• Provide education and guidance to nursing staff regarding documentation and MDS requirements.
