Glenwood Skilled Nursing & Therapy is seeking a PPS Skilled Case Manager / MDS Coordinator in Glenpool, OK. This combined role covers skilled case management and the full MDS process within a Nursing department.

The position is full-time, day shift, non-exempt, with a $2,500 sign-on bonus. The role assesses residents, plans care, coordinates payer requirements, and ensures regulatory-compliant documentation and accurate reimbursement.

Qualifications

• Current Oklahoma RN or LPN license.
• Associate degree with nursing licensure/certification or BSN.
• Knowledge of MDS, RAI process, care planning, and long-term care requirements.

Responsabilités

• Coordinate and oversee the complete MDS process including scheduling, data collection, completion, review, and electronic submission.
• Complete and coordinate required MDS assessments on time.
• Ensure MDS reflects current medical condition and care needs.
• Maintain MDS and Care Plan tracking for timely completion and compliance.
• Develop individualized, comprehensive care plans.
