Job summary

Boston Senior Home Care is seeking a Case Manager to conduct home visits in the Greater Boston area, assess elder eligibility for the State Home Care Program, and coordinate care plans with EOAI guidelines.

After developing and implementing the initial care plan, you will monitor the client caseload through home visits and phone contact, collaborating with informal and formal supports, vendors and community agencies.

Qualifications

• Bachelor's degree in social work or related field is preferred.
• LSW/SW credentials and relevant experience preferred but not required.
• Ability to obtain CORI clearance and work in the United States.

Responsibilities

• Conduct home visits to assess elder eligibility for state home care services.
• Develop care plans based on individual needs and EOEA guidelines.
• Implement care plans using BSHC resources and coordinate with supports.
• Maintain client records with timely documentation of communications.
• Monitor client caseload through visits and phone contacts; revise plans as needed.
• Advocate for clients and collaborate with vendors and community agencies.
• Engage in outreach to ensure access to services for all client groups.
