Job summary

Community Care, Inc. is seeking a qualified care coordinator to support elderly and disabled adults within our program. You will conduct assessments, develop care plans, and coordinate services to meet client needs.

Responsibilities include home visits, connecting clients with community resources, and maintaining thorough documentation. A bachelor’s degree in a related field and experience with older adults are required, with Spanish bilingualism preferred.

Qualifications

• Bachelor’s Degree in Social Work, Human Services, Psychology, or another related field.
• At least one (1) year of formal (non-family setting) experience working with a geriatric population in community care management or related community experience with older adults.
• Excellent interpersonal and communication (written and spoken) skills and the ability to work in a multicultural environment.
• Exceptional organizational and multitasking skills.
• Must be dependable, flexible, detail-oriented, and have basic computer knowledge.
• Bilingual Spanish speaker is highly preferred, but not required.

Responsibilities

• Coordinate and manage care for elderly and adults with disabilities within the Community Care, Inc. program.
• Perform initial and ongoing assessments including care plan development, risk reduction strategies, and service coordination.
• Identify, evaluate, implement, and monitor long-term healthcare needs and manage services.
• Maintain clear and updated documentation of all contacts and services provided.
• Perform home visits and maintain a regular schedule of client contacts and visits.
• Provide access to community resources for clients and families.
• Coordinate contact with family members on needs and services offered to a client.
• Coordinate and authorize services with service providers to meet identified client needs.
