Job summary
Mercyhealth Visiting Nurses Association, Inc. is seeking a Case Manager to perform comprehensive home and facility assessments, determine CCP eligibility, and develop care plans for participants and caregivers.
You will administer MMSE and DON, coordinate referrals to available benefits/services, and manage Medicaid applications when indicated. The role includes weekend on-call duties per IDOA and timely documentation to support evolving participant needs.
Qualifications
- Minimum 1 year experience in needs assessment and care planning.
- RN/BSN or related BA/BS required.
- One year of program experience may substitute for college.
- LPN with program experience may replace a baccalaureate degree.
Responsibilities
- Perform comprehensive assessments in participant's home or facility to assess CCP needs.
- Administer and score MMSE and DON and collect financial information.
- Create care plans and coordinate referrals for benefits/services.
- Complete Medicaid applications when indicated.
- Rotate weekend on-call duties as required by IDOA.
- Respond to inquiries and complete timely documentation and redeterminations.
