Boston Senior Home Care is seeking an Admissions Case Manager to support elders in the Greater Boston area by assessing eligibility, enrolling in state home care programs, and linking to community resources. The role involves developing and implementing comprehensive service plans with interdisciplinary input.

The position requires a BS/BA, willingness to travel to client homes, and strong communication skills.

Formación

• Requires BS or BA degree; licensure preferred.
• Strong communication and interpersonal skills are essential.
• Proficiency with MS Office and data entry is required.

Responsabilidades

• Conduct home visits to assess, evaluate and enroll elders for state home care services or make recommendations for community assistance.
• Develop a care plan based on unmet needs and ensure interdisciplinary review.
• Present new cases for approval and guidance to the Admissions Review Team.
• Implement the plan using BSHC and community resources.
• Maintain consumer records and document significant communications.
