Job summary

Boston Senior Home Care is seeking a Case Manager fluent in English and Haitian Creole to conduct home visits in the Greater Boston area. The role evaluates elder eligibility for state home care and coordinates care plans with community resources and supports.

Qualifications

• Bachelor's Degree in social work or related field preferred.
• SW, LSW and relevant work experience preferred.
• Bilingual English/Haitian Creole required.

Responsibilities

• Conduct home visits to assess eligibility for state home care services.
• Develop a care plan based on EOEA guidelines and unmet needs; ensure interdisciplinary review.
• Implement the plan using BSHC community resources.
• Maintain client records with timely documentation of communications.
• Coordinate with informal supports, vendors and community agencies.
• Conduct ongoing outreach to ensure access to services for all client groups.
