Job summary

AccessHealth in Brookshire, TX is seeking a Community Health Worker to support patients in achieving a Primary Care Medical Home and improving access to services. You will link patients to community resources and act as a liaison between patients and health/social services.

The role involves conducting intakes, visiting clients at clinics or in their homes, and documenting activities, with a focus on advocacy, outreach, and ensuring continuity of care for underserved communities.

Qualifications

• Two years of customer service experience.
• Able to operate a computer, fax, and scanner.
• Bilingual English/Spanish preferred.

Responsibilities

• Screen referrals to determine the best program for the patient.
• Help patients access resources, including food, health insurance, housing, transportation, and health information.
• Follow up with patients to ensure access to medical care.
• Link clients with community resources and referrals to outside agencies.
• Document activities and assistance in the client’s chart in a timely manner.
• Meet with clients in clinics, homes, or hospitals as needed.
• Prepare weekly client reports and maintain program rosters.
• Comply with company policies and regulatory requirements.
