Job summary

UnitedHealthcare is seeking a Field Based Community Health Worker in the North Olympic Peninsula region (Western Washington). The role focuses on engaging Medicaid members, conducting home and community assessments, and delivering education to improve access to care.

The CHW works primarily in the community, spends 50–75% of time in the field, and may telecommute for rural regions. Requires HS diploma and WA residency; travel 50–75% and occasional evenings/weekends are expected.

Qualifications

• High School Diploma or GED required
• Experience engaging in local community resources
• Resident of Washington state
• Designated workspace with internet access
• Ability to travel 50-75% of the time
• Availability Mon-Fri 8am–5pm, some evenings/weekends
• Reliable transportation and valid US driver's license
• Telecommuting may be possible if located in Rural/Tribal Regions (Western Washington)

Responsibilities

• Create a positive experience and relationship with the member
• Proactively engage the member to manage their own health and healthcare
• Provide member education on community resources and benefits
• Support member to engage in work or volunteer activities and build social supports
• Engage members face-to-face or by phone for follow-up
• Drive to member homes when needed to reach otherwise hard-to-reach populations
• Conduct outreach to promote health programs and services
• Coordinate with teams to ensure quality performance
• Maintain detailed records and coordinate with care managers
• Understand local Medicaid/Medicare resources and community cultures
