Qualifications

• 3+ years of experience in a healthcare role.
• Proficiency with Microsoft Outlook, Teams, Excel, Word, and multiple web apps.
• Ability to travel to visit Medicaid members, up to 25% of time.
• Must reside in Nebraska; Nebraska drivers license and auto insurance required.
• Stable home internet with UnitedHealth Group approved high-speed connection.

Responsibilities

• Assess, plan, and implement individualized care strategies for members.
• Create positive member relationships and coordinate with care team and resources.
• Identify referrals for social services and manage care plan across continuum.
• Document assessments, care plans, and referrals accurately.
• Engage members in person or by phone and meet documentation requirements.
