Job summary

San Diego American Indian Health Center is seeking an ECM Supervisor to oversee day-to-day operations of the ECM program, supervising the care team and managing caseloads to ensure timely assessments and care plans. The role supports compliance with DHCS, HIPAA, and HRSA requirements while partnering with MCPs and community providers.

The ideal candidate brings 3+ years in care coordination or case management, plus supervisory experience, and a bachelor’s degree in a related field.

Qualifications

• Bachelor’s degree or higher in a related field with 4+ years of related experience.
• Minimum 1 year of supervisory or lead experience in a health setting.
• Experience with Medi-Cal/CalAIM ECM and HIPAA requirements preferred.

Responsibilities

• Supervise ECM Lead Care Managers, care coordinators, and support staff; assign caseloads and schedules.
• Oversee ECM operations to ensure timely assessments, care plans, and documentation for billing.
• Coordinate with MCPs, primary care, behavioral health, and community partners for whole-person care.
