Job summary

Central Neighborhood Health Foundation is seeking an ECM Supervisor to direct and manage the Enhanced Care Management program. You will oversee Lead Care Managers and Community Health Workers, ensure workflows, track deliverables, and support upper management with data-driven insights.

You will coordinate with care teams, partners, and funders to maintain program quality, staff training, and policy compliance while driving improvements in care outcomes.

Qualifications

• Bachelor’s degree in health or related field or equivalent combination of training and experience.
• Minimum 2 years of recent progressively responsible administrative management/supervisory experience.
• Project management experience with timeline, work plan oversight and development.
• Experience in nonprofit case management for people with disabilities and chronic health conditions.
• Experience managing multidisciplinary care teams.

Responsibilities

• Direct day-to-day ECM staff operations including supervision.
• Develop professional relationships and maintain accountable workforce.
• Coach staff and administer disciplinary actions when needed.
• Collaborate with internal/external stakeholders to improve care quality and workflow.
• Oversee member case files, review/audit files and provide feedback to staff.
• Analyze program data and generate reports on objectives.
• Support Program Director in daily operations and staffing plans.
• Ensure staff training and grant/department compliance.
