Job summary

Amity Foundation seeks an ECM-Program Coordinator to lead the Enhanced Care Management-Community Supports program in the assigned county. You will supervise staff, ensure contract compliance, and coordinate resources to improve care outcomes for high-need students.

The role requires strong communication, stakeholder engagement, and budget oversight, with a focus on collaboration with partners and continuous program improvement.

Qualifications

Responsibilities

• Oversee ECM-CS program development and implementation in the assigned county.
• Supervise assigned staff and ensure program compliance with contracts and regulations.
• Train multi-disciplinary staff and coordinate clinical and non-clinical resources.
• Foster collaboration with healthcare providers and community partners.
• Oversee billing/claims processes and monitor program operations for improvements.
