Job summary

Qualifications

• CA RN license required; BSN preferred.
• Acute care experience preferred.
• Care management and managed care experience preferred.
• Bilingual Spanish preferred.

Responsibilities

• Provide day-to-day clinical oversight of ECM team members.
• Collaborate with interdisciplinary team to analyze member outcomes and utilization of ECM/Community Supports.
• Ensure adherence to DHCS CalAim regulatory requirements, CMD Model of Care, and contract deliverables.
• Advocate for member health outcomes, health equity and care coordination standards.
