Job summary

Pacific Health Group is seeking an ECM Lead Care Manager Team Lead to support frontline operations and mentor LCMs. You will help maintain workflows, provide coaching, and ensure high-quality, person-centered care delivery across the department.

The role blends field-based member support with administrative leadership. The position emphasizes mentorship, problem-solving, and improving outcomes for vulnerable populations while balancing direct member care with operational responsibilities.

Qualifications

• 3+ years of ECM, case management, or healthcare coordination
• Experience mentoring or training staff
• Strong knowledge of Medi-Cal, CalAIM, and ECM requirements
• Excellent communication and leadership skills
• Ability to balance member care with operational duties
• Strong documentation & technology proficiency
• Experience with vulnerable populations preferred

Responsibilities

• Provide day-to-day support and guidance to fellow Lead Care Managers (LCMs)
• Serve as a resource for staff questions, workflows, and ECM best practices
• Monitor team productivity, documentation completion, and operational consistency
• Identify and elevate concerns, risks, workflow gaps, or trends to management
• Support coverage needs, including PTO coverage
• Collaborate with leadership to maintain accountability and quality of care
• Reinforce workflows and compliance standards across the team
• Support onboarding and development of new and existing LCMs
• Provide shadowing opportunities during in-person visits and fieldwork
• Demonstrate best practices for member engagement and care coordination
• Provide real-time coaching and operational support
• Reinforce documentation standards and QA initiatives
• Maintain compliant documentation per Medi-Cal, CalAIM, ECM requirements
• Support quality improvement and internal auditing processes
• Ensure care coordination activities are properly documented
