Job summary

Qualifications

• Bachelor's degree preferred or equivalent work experience in Social Services/Public Health/Behavioral Sciences.
• LVN license required; must maintain LVN with the State of California.
• Ability to communicate effectively with clients and partners in the community.

Responsibilities

• Outreach in public settings to identify Medi-Cal eligible individuals and enroll in ECM services.
• Perform comprehensive assessments and create individualized care plans to support health and social outcomes.
• Provide care coordination and navigation to healthcare, behavioral health, and housing services.
• Accompany members to appointments to support engagement and access to services.
• Assist with SSI documentation and benefits advocacy as needed.
• Identify resources for housing (HUD/Section 8) and aid in housing placement.
• Maintain caseload management with guidance on caseload size and productivity standards.
• Proficient data entry, reporting, and data analysis; meet productivity standards (62%).
• May support additional duties related to the position.
