The role includes developing partnerships, ensuring contractual compliance, conducting home visits, and guiding staff through training plans while maintaining HIPAA compliant documentation.

Qualifications

• Valid Clinical Social Worker License (LCSW/ACSW) with the State of California.
• 1 year of experience in case management.
• 1 year of experience working with underserved patient populations.
• Valid Driver’s License with clean/good driving record
• Clean fingerprinting/livescan

Responsibilities

• Oversee ECM benefit management and care delivery models.
• Lead partnerships and interdepartmental collaboration.
• Conduct staff meetings and training with ECM Director.
• Develop staff training plans and performance reviews.
• Support home visits and participant assessments.
• Ensure documentation complies with HIPAA and privacy laws.
