Job summary

Sacramento Native American Health Center, Inc. seeks an ECM Care Manager to deliver direct services and care coordination within a multidisciplinary team.

Hybrid role; in-office for the first 90 days. You will assess physical and mental health, SUD, housing, and social needs, develop ICPs and HAPs, coordinate with providers with consent, and document clearly in the EHR while upholding HIPAA and cultural relevance.

Qualifications

• Prior experience in care coordination or case management is required.
• High school diploma or equivalent is required.
• Familiarity with local community resources and support services.
• Strong interpersonal skills, empathy, active listening, sound judgment and teamwork.
• Proficient in Microsoft Office, Office 365, Teams, Zoom and comfortable with EHRs.

Responsibilities

• Assess client needs across physical health, mental health, SUD, housing and social supports.
• Develop and oversee Individual Care Plans (ICP) and Health Action Plans (HAP).
• Provide services in-office, via telehealth, and in the field.
• Coordinate care with providers (with consent) and maintain continuity from assessment through discharge.
• Apply evidence-based approaches (Motivational Interviewing, Trauma-Informed Care, Harm Reduction).
• Document services clearly, timely and compliantly in the EHR.
• Attend required meetings/trainings and uphold professional conduct and HIPAA.
